What are the acceptable forms of employment for employment verification?

These are the acceptable documents to prove employment:

 Your most recent paystub:

 You may verify employment by sending a copy of a paystub. Your verification document must include:

  • Your full name
  • Retail chain details (name, store number and address)
  • The date (must be from the last 30 days)
  • Note: black out all sensitive data such as your Social Security Number or bank account number

 Or

 Employment verification letter:

You may verify employment with a letter from your supervisor or HR representative. All employment verification letters must include the following information:

  • Supervisor or HR representative verification that you work at the store specified on the claim form
  • Supervisor or HR representative contact information (name, title, company email and phone number)
  • Letter on company letterhead (preferred)
  • The date (must be from the last 30 days)
